2010 ASA Registration

Please fill out preformated form and save to your desk top.  Email a copy of this form to me at asaomaha@yahoo.com .  ONLY ELECTRONIC COPIES WILL BE ACCEPTED. NO HANDWRITTEN FORMS. Then make check payable to "Vince Stephens" and mail to me at 2803 Hunter Drive, Bellevue, NE 68123.  Cost is $16.00 per player/coach.   At minimum of one coach has to be on each form.

ASA Championship Roster

When printing the ASA Championship Roster it needs to be printed on one sheet of paper (front and back). Two sheets will not be accepted as a roster.

Championship Tournament Entry Form

This form is required for teams that are entering Championship Qualifying tournament. This form requires your ASA Commissioner signature and then mailed with payment to respective tournament director.

 ASA Pickup Player Form

The Pick Up Player Form can only be used after the Final Qualifying Tournament has been played (useally Nebraska National Qualifier or Nebraska State Qualifier) .  Players can only come from teams that are no longer eligible for Championship Play.  Pick up Players must have been on an ASA roster in the current year and must come same division of play or lower.  All ASA 18A players and below have to use Nebraska ASA registered players only.  Gold Level can use players from Region 12.
