For Omaha ASA Registrations: 1. Download Omaha ASA Registration form 2010 ASA Registration Form to your desktop. (Please do not send 2010 Registrations until after 15 Jan 2010) 2. Type out your registration information into the automated form. 3. Ensure at least one coach is registered on your registration form. 4. Cost is 16.00 per player/coach. 5. Ensure all your contact info is correct, with correct age group and classification is filled out completely. This includes valid email address. 6. Save a copy to your desktop and email that file to me at asaomaha@yahoo.com I will only accept electronic copies. NO HANDWRITTEN FORMS. 7. Make payment to "Vince Stephens" . Please include team name in memo section and mail payment to me at the below address: Vince Stephens 2803 Hunter Drive Bellevue, NE 68123 8. It takes about two weeks to receive your ASA Insurance Cards from the State Office. 9. You will receive a 2010 ASA Rule Book, and Score Book.
